QUALITY CARE – At Livingston Health and Rehabilitation Center, we provide a continuum of care that bridges the gap between hospital and home. Whether you are in need of short-term rehabilitation or long-term care, our team works to help you recover and return home. We are pleased to offer outpatient therapy for those who no longer need inpatient therapy but may benefit from a schedule of therapy treatments.  With our Commitment to Caring, we are a 115-bed licensed facility offering full therapy & rehab services.  All beds are Medicare certified.

COMPREHENSIVE EYE EXAMS – Our two-part comprehensive eye examination with one of our optometrists evaluates the vision and the health of your eyes.  We provide eye exams for all ages starting as early as 6 months.  Early detection of potential eye health and vision problems for our patients is of the utmost importance.  Ask about family scheduling options.

EYEWEAR & CONTACT LENSES – Our optical area has many options to choose from including glasses, sunglasses, contact lenses, and many optical accessories.

The One Health ( CHP) Livingston Medical Clinic has been offering primary and preventative medical care to patients of all ages as well as behavioral health for adults since 1997.   Services are provided by experienced family physicians, physician assistants, licensed clinical professional counselors, and licensed clinical social workers.   A resource coordinator/certified application counselor is also available to assist with referrals, help with paperwork, accessing public benefits, and insurance on the Healthcare Marketplace.   Sliding scale fees are available to qualifying patients.
